School Transport Declaration Form (2026–2027)

I/We hereby accept the following terms with regard to the School Transport co-ordinated by St. Catherine's High School, Pembroke, and operated by Keith Borg as the chosen School Transport Operator.

  1. I/We understand that the School Transport provision is unsupervised by School staff members and that I/we assume sole and full responsibility for the welfare and behaviour of my/our child when making use of such service.
  2. I/We agree to co-operate with the School in its efforts to provide a good service:
    1. I/We understand that the School will not accept any requests directly from students saying that they are not taking the transport or wanting to take another route.
    2. I/We am/are obliged to inform the School when my/our child/ren will not be making use of the School Transport on the return trip (from School to home). This does not apply in the case of my/our child staying after school hours for the After-School Programme.
    3. I/We understand that the School will not accept any requests for my/our child/ren to change to any other routes (other than those routes which have been registered at the application stage) for no particular reason.
    4. I/We agree that such notification/requests (as per clauses 2a and b) are to be made by sending an email (on transport@stcatherines.eu) by not later than 10.00pm of the day before. Alternatively, such notification/requests can be made to the School Receptionist, Ms. Marthese Vella on the day, in writing and duly signed by the parents, by not later than 10.00am.
    5. I/We will inform the driver if my/our child/ren will not be attending school on a particular day and, likewise, when the child/ren will be returning to school. I/We agree that the only communication with the driver should be by phone communication. Text messaging the driver will not be an acceptable means of communication.
  3. I/We agree that no arrangements will be made directly with the drivers unless permission is first sought from Mr Antoine Muscat.
  4. I/We agree that my/our child/ren will wear the School uniform when travelling to and from school, and they will behave in an appropriate manner. In the case of misbehaviour or dangerous conduct taking place on the van, the School reserves the right to address such issues with disciplinary action as it (and/or the driver of the particular route) deems fit. This may also lead to temporary suspension or permanent exclusion from the use of transport provision.
  5. I/We understand that the School Transport is being co-ordinated as an added service for the convenience of parents and students but the School is not responsible for the actual transporting of students to and from school.
  6. I/We understand that, in the event that the transport route is not operative due to a vehicle breakdown, the School or School Transport Operator will do their utmost to inform me/us in due time. However, I/we understand that the School is not to be held responsible to provide and/or pay for alternative means of transport in such eventuality. I/We understand that it is my/our responsibility to bring my/our child/ren to school in such extraordinary circumstances.
  7. I/We understand that, in the event of a traffic accident, the School is not held liable, medically or financially, for any injuries incurred by my/our child/ren.
  8. I/We understand that, should an accident arise, the School / School Transport Operator will do their utmost to contact the parent/s. In the eventuality that I/we cannot be contacted, I/we give my/our consent so that emergency treatment/intervention (which, unless administered at that point in time, would put the child in danger) may be given to my/our child.
  9. I/We understand that Application Forms will be processed on a first-come-first-served basis and that, therefore, late submissions may result in exclusion from School Transport provision due to insufficient seating capacity or insufficient routes available.
  10. I/We understand that the School Transport co-ordinated by St Catherine's High School falls under the terms and conditions of the Government Scheme for Free School Transport. Furthermore, I/we understand that these terms and conditions may change without prior notice due to extraordinary circumstances.
